Building a Healthier, More Active Putnam County
The Creating Healthy Communities (CHC) grant program at the Putnam County Health Department is a long-term initiative focused on reducing chronic disease and advancing health equity by making lasting changes to our community’s built environment. Funded by the Ohio Department of Health, the CHC program works to improve access to physical activity and healthy foods—especially in communities experiencing the greatest health disparities.
What We Do
Through the CHC grant, the Health Department leads several evidence-based initiatives in partnership with local organizations, leaders, and residents. These initiatives are driven by community input and guided by comprehensive assessments that identify opportunities to improve the health of our towns and neighborhoods.
Our Coalitions: Community-Driven Change
We facilitate Active Living and Food Coalitions in both Ottawa and Leipsic. These coalitions are made up of diverse local stakeholders—including residents, schools, businesses, nonprofits, and local government—and are open to anyone who wants to join.
The goal of these coalitions is to:
- Identify community strengths, assets, and needs
- Engage individuals with lived experience
- Prioritize health-related projects based on community feedback
- Guide both short-term improvements and long-term planning for the next four years
