If you or your child have received vaccinations from another healthcare provider, please bring a copy of your immunization record to your appointment.

Vaccines for Children (VFC) Program

The Vaccines for Children (VFC) program is a federally funded program that ensures all children have access to vaccination.

Who is Eligible?

Children through 18 years of age who meet at least one of the following criteria are eligible to receive VFC vaccines:
Medicaid eligible, uninsured, American Indian or Alaska Native, underinsured.
