The Putnam County Health Department accepts most major health insurance plans. We recommend contacting your insurance company to confirm that we are an in-network provider and to verify your coverage details.
If you or your child have received vaccinations from another healthcare provider, please bring a copy of your immunization record to your appointment.

Clinic Hours – by appointment only:
- Tuesdays From 9:00 – 11:30 AM & 1:00 – 4:00 PM
- Extended Hours On The Second & Fourth Tuesday Of Each Month Until 6:00 PM
- Immunization Clinic On The 4th Wednesday Of The Month (9:00-11:30 AM)
- Immunization Clinic Hours May Change During Holiday Months

Vaccines for Children (VFC) Program
The Vaccines for Children (VFC) program is a federally funded program that provides vaccines to child who might not otherwise be vaccinated because of inability to pay.
Who is Eligible?
Children through 18 years of age who meet at least one of the following criteria are eligible to receive VFC vaccines:
Medicaid eligible, uninsured, American Indian or Alaska Native, underinsured.
