Death records

Certified death certificates are available for any person who died in Ohio beginning in 1909. Certified copies of death records are $27.00 each.

Obtain a Death certificate

In Person

A completed request form and payment is required. Cash and personal checks are accepted. Debit/credit cards are also accepted (additional fees apply).

Death certificates are available at the Putnam County Health Department

Monday through Friday,

8:30a.m. – 4:00 p.m.

These are usually available within a few minutes.

By Phone

Birth and death records may be requested by phone by calling 419-523-5608.

A credit card is required for payment and additional charges for credit card use will apply.

By Mail

Complete the request form and include the signature of the person who is purchasing the certificate. Mail the completed request form along with a check or money order and a self-addressed, stamped business-size envelope. Mailed requests are usually processed the same day they are received. Please mail to: Putnam County Health Department, 256 Williamstown Road, Ottawa, OH 45875. Checks payable to “Putnam County Health Department.”
